AM-PAC "6-Clicks" Basic Mobility

Scoring Scale: Each of the 6 items is scored from 1 to 4 based on the level of assistance required:

1 = Unable / Total assistance (dependent)
2 = A lot of assistance (moderate-to-max assist)
3 = A little assistance (min assist or supervision)
4 = None (completely independent)

Raw Basic Mobility Score (6–24)

High Risk (< 14)

Strongly correlates with severe functional dependency, high caregiver burden, and increased risk of hospitalization or institutionalization (if using in acute care setting: strongly correlates with increased odds of needing a post-acute care facility [SNF/IRF]).
